SOCIOLOGY OF COMMUNITY NETWORKS AND ORGANIZATION OF PRIMARY CARE
The module aims to use systems analysis to "read" and analyze the reasons healthcare organizations that are leading the health care system in Europe and therefore also of Italy to focus on primary health, or community as a model response to the changing health needs and social health of the community due to the epidemiological transition ( transition from infectious disease to the chronic degenerative ), the demographic transition (declining birth rates, an aging population, the increase of immigrants), and finally the social transition (contraction of households, reducing network parental precariousness of the social network)
COMMUNITY NURSING AND THERAPEUTIC EDUCATION
The module aims to describe the stages that led to the paradigm shift from biomedical model to the biopsychosocial linked to the disease that leaves room for social, psychological and behavioral related disease . In this logic will place the nursing community as an appropriate response to the health needs of nursing and community health and social
Will be presented with the community nurse with the characteristics and skills that make it recognizable in our reality related to the development of case management, taking charge early and the design of integrated care plans.
Among those skills that will be presented to develop a plan of therapeutic education is to maintain a continuity of care and to develop the ability " to self " .
ORGANIZATION OF COMMUNITY
The module examines the assistance of community by entering it into the logic of primary care with the characteristic features that define it and include the leading centers of primary care (home, family, informal and formal networks, residences houses of health, etc. ), the mode of delivery (inter- integrated networks, systems and spoke hab etc. ) and the time health and social health finally the mode of operator training to the community.

- SOCIOLOGY OF COMMUNITY NETWORKS AND ORGANIZATION OF PRIMARY CARE + COMMUNITY NURSING AND PATIENT EDUCATION + ORGANIZATION OF ASSISTANCE OF COMMUNITY
The oral exam is integrated on the main contents that emerged in the three courses; each student it is also required of a welfare case solution through integrated care planning in the community. The time is about 20 minutes for each student, are evaluated accuracy of the content, capabilities to solve problems, use of scientific language and integration with other disciplines; the valuation is thirty.
THE FINAL GRADE IS A WEIGHTED AVERAGE OF THE MARKS OBTAINED IN EACH TESTS. Reference texts
