Training objectives

EXERCISE IN NEUROLOGICAL DISEASES
Brief presentation of the main pathological dysfunctions of the neuro-muscular and central nervous system that limit reflex, automatic, voluntary and psycho-motor physical activity of the child, the adult and the elderly. Proposals for prevention and rehabilitation activities.
EXERCISE IN RHEUMATIC DISEASES
Knowledge, understanding and learning skills about the main clinical and classification aspect of the most relevant rheumatic diseases with emphasis on the alert signs and symptoms ( “Red flags”) which can occur during motor exercise with particular attention to mechanic and inflammatory back pain, musculoskeletal pain, fibromyalgia and osteoporosis.
To achieve the capability to autonomously evaluate each single patient and to recognise his/her most relevant needs related to the different phases of the ongoing disease and to plan and communicate to the patient the most appropriate and personalized exercise program (making judgements and communication skills).

Prerequisites

Basic knowledge of anatomy and physiology of the nervous system and of the muscular-skeletal apparatus.

Course programme

EXERCISE IN NEUROLOGICAL DISEASES
General information on disorders of the neuro-muscular system
- The primary muscle disorders
- Disorders of the neuromuscular junction
- The hyperactivity of the motor unit: tetanus, cramps
The pathology of peripheral motor neuron: defects of trophism, tone, muscle strength, reflex activity.
Sensory system: an outline.
- Disorders of motor function from dysfunction of the sensory system: sensory ataxic syndromes, nociceptive and neuropathic pain.
Disorders of motor function from impairment of the pyramidal tract in brain and spinal cord pathology: central paralysis, spasticity. Pharmacological and physical treatment strategies.
Autonomic nervous system disorders and their effects on motor functions.
Functional exploration of ways and sense of motion through the electrophysiological strategies:
- Electromyography.
- Motor and sensory conduction velocity of nerves.
- Event-related evoked potentials (motor, sensory)
Disorders of cerebellar function: cerebellar ataxia, dysmetria.
Disorders of vestibular and vestibular-cerebellar function.
Movement disorders from dysfunction of extrapyramidal system: the hypokinesia -bradykinesia in parkinsonian syndromes, rigidity-plasticity system failures and postural control in parkinsonian syndromes
- The abnormal involuntary movements. Disorders of the main higher brain functions related to motor activity: body image disorders, disorders of psycho-motor functions, with special reference to apraxia, cortical ataxic syndromes
Major disturbances of standing and walking dysfunction of the nervous system and spinal-muscular system
Gender issues in neurological disorders.
EXERCISE IN RHEUMATIC DISEASES
The course forecasts 12 hours of teaching (frontal lectures)
Classification of the main rheumatic diseases (2 hours).
Basic information about clinical and semeiology aspects of the main inflammatory (rheumatoid arthritis, seronegative spondyloarthritis) (4 hours) and degenerative (osteoarthrititis, osteoporosis) (4 hours) rheumatic disease
Basic information about clinical and semeiology aspects of the main connective tissue diseases (2 ore)
Mechanic and inflammatory Low Back Pain (2 hours)
Localized and generalized (fibromyalgia) extra-articular rheumatisms (2 hours)
Physical activity and rheumatic diseases (2 hours)
Aspects of joint economy (2 hours)

Didactic methods

Frontal lessons. Training at rehabilitation centres, gymnastics and other structures which are dedicated to promotion of adapted motor activities for neurological patients.
Optional participation at seminars and workshops, advanced courses, meetings on neurological movement disorders.

Learning assessment procedures

EXERCISE IN NEUROLOGICAL DISEASES
Oral examination. Two questions on the following topics:
-Semeiology of the Diseases of the Nervous System generating movement disorders
-Strategies of adapted motor activities in neurological disorders.
Duration of exam is 20 minutes. Knowledge, clarity of presentation, communicative abilities, logical speaking, capability of connection and application to professional practice.
EXERCISE IN RHEUMATIC DISEASES
Written test consisting of 30 multiple choice questions each of them providing 4 possible answers (only one is right). The final score is calculated attributing 1 point for each right answer, 0 points in case of wrong or not provided answer. The final mark is the sum of the points achieved. The time allowed for this test is 1 hour. It is not allowed consulting any textbook or using any PC, smart phone, calculator…. To pass the exam it is necessary to get at least 18 point out of 30. In the event of achieving 30 points it is also offered an additional question to improve the final mark with laude (in case of right answer). If the tests is not passed (final mark < 18), it is necessary to repeat the exam in a subsequent oral format.
THE FINAL GRADE IS A WEIGHTED AVERAGE OF THE MARKS OBTAINED IN EACH TEST.
